Curious as to why some of my produce has large tops and small bottoms. The garlic has rather large stalks and small garlic heads. Fertilize new hydrangea

we are putting in a new Everlasting Revolution Hydrangea (Macrophylla Hokomareva). Paper work says fertilize with a phosphorous-higher NPK. We have Osmocote Smart Release for Azalea-Camellia-Rhododendron. Is this OK for the hydrangea? Thanks!
