Phosphate based fertilizer

Sick Helleborus

My helleborus is struggling

After four years of blossoming on my son's birthday in February, my helleborus is struggling to survive. I don't know how to include a picture, but the leaves are multicolored and look like they have diseased. I have added lime to the soil. Should I replace the plant? Could it be voles eating the roots? What do you think?
